Over the past two years, I've seen one of the biggest shifts in the aesthetic industry that I've experienced in my entire career. The revolution is happening because of GLP-1 weight-loss medications like Ozempic (semaglutide), Wegovy (semaglutide), and Mounjaro (tirzepatide), which are delivering remarkable results for many patients.
As more patients in Austin and across the country lose 20, 40, even 70+ pounds on these medications, they're coming into my office with a unique set of goals and challenges. This has created a new category I now see daily — what many are calling the GLP-1 makeover.
Why GLP-1 Patients Need a Different Surgical Approach
- Weight comes off quickly — rapid loss creates more pronounced skin laxity in the abdomen, arms, neck, and thighs
- Muscle mass may decrease — without careful nutrition, some patients lose lean mass, changing how skin drapes and how tissues support healing
- Collagen quality may be reduced — affecting skin elasticity, incision strength, scar quality, and healing speed
- Nutrition isn't always optimal — GLP-1 medications lower appetite profoundly, which can create vitamin and protein deficiencies critical for post-surgical healing
The Most Common GLP-1 Makeover Procedures I Perform
Tummy Tuck (Abdominoplasty)
Still the #1 request. Rapid weight loss often leads to loose abdominal skin, widened muscle separation, and lower abdominal laxity.
Arm Lift (Brachioplasty)
The upper arms are extremely sensitive to weight fluctuation. Patients often notice "batwing" deflation. With very high weight loss, excess can extend into the forearm, requiring more aggressive techniques.
Thigh Lift
Inner thigh laxity tends to be more pronounced with medication-driven weight loss. Inner thigh lifts have scars in the groin crease; lateral thigh is lifted as part of a lower body lift.
Breast Lift or Lift With Implants
Rapid weight changes often require a lift for shape/position, an implant for restored volume, or both. Fat transfer can sometimes achieve volume increase without implants.
Lower Body Lift
For patients losing 50–100+ pounds, a circumferential body lift may be needed — combining a tummy tuck with liposuction and lifting the back/buttock/thigh region. This is one of the most aggressive procedures we have to restore hourglass contours.
Challenges I See — and How I Address Them
Healing May Be Slower
We recommend increasing protein intake around surgery and supplementing with multivitamins. GLP-1 medications must be held 2 weeks prior to surgery (current guidelines say 1 week, but for elective cosmetic surgery we use 2 weeks) to minimize aspiration risk with anesthesia.
Scar Quality May Be Less Predictable
We use multiple layers of longer-dissolving sutures (PDS), external scar support (Steri-strips, Sylke, Brijjet devices), close post-op supervision, steroid injections if needed, and microneedling or CO2 laser treatments.
Thin, Deflated Tissue Areas
We may use fat grafting (transferring fat from areas of excess to thin areas) or Morpheus8/CO2 laser to thicken skin via collagen stimulation.
If You're Just Starting GLP-1 Medications
- Drink lots of water
- Preserve muscle mass with resistance training — compound movements (bench press, rows, squats, leg press) help stimulate growth hormone
- Get enough protein daily
- Don't worry about excess skin — we can address it
