What Is a Deep Plane Facelift?

A deep plane facelift — also called a rhytidectomy — is the most advanced form of surgical facial rejuvenation available. Unlike a traditional or mini facelift, which only tightens the overlying skin, the deep plane technique releases and repositions the SMAS layer (superficial musculoaponeurotic system) and the ligaments that anchor the face's soft tissues to the underlying bone.

This structural repositioning is why deep plane results look naturally refreshed rather than tight or "pulled." The face isn't stretched — it's lifted back to where it was a decade or two ago. Jowls, deep nasolabial folds, midface descent, and neck laxity are addressed simultaneously in a single, comprehensive procedure.

A traditional facelift tightens skin only, which can look tight or windswept and tends to last about 5–7 years. In a deep plane facelift I release and reposition the SMAS layer and the deep facial ligaments — the actual structural foundation — so the result looks naturally younger and lasts more like 10–15 years. The trade-off is a slightly longer surgery and recovery, which most of my patients feel is well worth it.
Yes — this is one of the things I'm known for. I perform my wide-awake deep plane facelift entirely under local anesthesia at my Austin surgical suite. That avoids the risks of general anesthesia (nausea, cardiovascular stress, airway issues), usually means less bruising and swelling, and makes for a faster, more comfortable recovery. Most people qualify, and I'll confirm you're a candidate at your consultation.
'Awake facelift' is just the common name for a facelift done under local anesthesia with light oral sedation instead of general anesthesia — and that's exactly how I do my deep plane facelift here in Austin. You stay comfortable and lightly sedated, breathing on your own, with no IV sedation and no general anesthesia. You get the same comprehensive deep plane result, with the added safety and faster recovery of staying awake.
Most patients rest at home for 10–14 days. Bruising and swelling peak around days 3–5 and settle a lot by the end of week 2, and most people are comfortable being out in public by 2–3 weeks. Your final, fully settled result shows up at 3–6 months as the last of the swelling resolves. The wide-awake technique tends to recover faster than general anesthesia.
I place the incisions within the hairline, in the natural crease in front of the ear, and behind the ear in the hairline — spots that hide in your hair and natural skin folds. Most of my patients find their scars are essentially invisible by 6–12 months. Good scar care, silicone sheeting, and sun protection while you heal all help you get there.
The best candidates are adults in good general health with noticeable sagging, jowling, deep nasolabial folds, and/or neck laxity. Non-smokers do best. If you have certain medical conditions or smoke heavily, you may need clearance and a tobacco-free period first, or may not be a candidate.
