A ‘tired’ upper lid is often a muscle problem, not just extra skin. Advancing the levator (tarsolevator advancement) resets the lid height.
Doing it wide-awake under local anesthesia lets me fine-tune the lid position with the patient participating — which is how you get a natural, symmetric result.
