This patient had a ruptured left saline implant and capsular contracture of the right breast for a couple of years, after a prior augmentation and lift, and was bothered by asymmetry, volume loss, and drooping. Dr. Levesque removed and replaced the saline implants with shaped silicone implants, built an internal bra from her own capsular tissue using a capsular flap in a new subpectoral pocket, and revised her old breast-lift scars. This capsular-flap technique is one Dr. Levesque learned during fellowship with Dr. Grotting and detailed in a chapter he authored in a breast-repair textbook.

Dr. Levesque on the Breast Revision

A ruptured implant on one side and a contracted capsule on the other is a classic revision challenge. Rather than discard the capsule, I raise it as a flap to build an internal bra in a fresh subpectoral pocket — it supports the new shaped silicone implant using the patient's own tissue.

Switching to shaped cohesive-gel implants restores symmetry and a natural slope, and revising the old lift scars cleans up the result. It's a technique I trained in with Dr. Grotting and wrote about in a breast-repair textbook chapter.
