This was a secondary facelift — she'd had one years before and had grown tired-looking again. A SMAS plication with lateral platysmaplasty re-tightened the deeper muscle layers of the face and neck, and structural fat grafting restored the volume that recurrent aging had thinned.

This case was performed under general anesthesia, which I still offer when a patient prefers it, though I now routinely do these in-office under local. She is shown early in healing, so some hairline incision remains visible and will settle over time.
