A constricted (tuberous) breast has a tight lower pole and short nipple-to-fold distance — you can't just drop an implant in, you have to release the constricting tissue and often lower the fold. Doing that by 1.5 cm is high-risk for a double-bubble, so it takes careful release and internal sutures to set the new crease.
A subfascial pocket lets you redistribute the narrow breast tissue over a wider base, and a moderate, round profile fills the lower pole she was missing. She kept a little native nipple asymmetry because we corrected shape with the implant rather than adding a mastopexy.
