This patient had a previous breast reduction with an inverted-T scar by another surgeon and grew dissatisfied over time. Dr. Levesque performed a comprehensive revision: a breast lift, a breast augmentation with 370cc HSC smooth round low-profile Sientra silicone implants in a subglandular position, and Durasorb mesh as an internal bra. The wider, taller, lower-projection implant fills the upper pole naturally without over-displacing the native tissue, the mesh supports the implant and improves scar outcomes, and the inverted-T scar was revised with repositioning of the nipple-areolar complex. Dr. Levesque on the Breast Revision

Revising a previous breast reduction is more complex than a primary case — the tissue has scar from the inverted-T, and the goal is to restore upper-pole fullness the patient lost. A wider, taller, low-projection implant fills the upper pole naturally rather than pushing the breast forward.

The Durasorb mesh acts as an internal bra: it supports the implant, offloads tension from the skin, and improves the scar over time. Combined with a lift and a revision of the old inverted-T scar, she has excellent early results.
