Revision cases like this are about problem-solving, not just adding volume. She came in asymmetric with scar indentations, so I chose different implant sizes side-to-side — 545cc left, 580cc right — to even out the projection and width rather than forcing identical implants onto unequal breasts.
The OviTex mesh acts as an internal bra to hold the implants in a symmetric position long-term, and 200cc of fat grafting on each side softens the implant edges and smooths the old scar indentations. Even challenging revisions can move a patient much closer to her goals with the right plan.
